- Then complete this Accident Form and turn it into the main office!
The following extracurricular activities are offered. The following forms and fees are required for participating students in grades 7 through 12 and must be turned in ASAP prior to the first scheduled game. Students who do not have paperwork turned in may not be permitted to compete. EVERY student needs to have a physical on file with our nurse’s office before they are permitted to practice or compete.
